AllinaHealth SeniorCareTransitions will join our campus as of 09-01-2017

We are happy to announce that AllinaHealth SeniorCareTransitions will now be on our campus!

The SeniorCare Transitions team is made up of specialists who understand the specific health care needs of seniors.  They specialize in:

Management of conditions such as arthritis, diabetes, congestive heart failure and denentia.

Help with recovering from strokes, fractures or surgeries

pain management

medication adjustments

If you choose to go with the Allina team, they will create an individualized health care plan with you.  They will meet with you within 30 days.  They will see you for routine appointments as well as urgent health care needs.  The team is available for visits during business hours and is on-call and available by phone 24 hours a day, 365 days a year.

Just like services provided at a clinic, a claim for services will be submitted to your insurance company.  They take most insurances.

There is no additional charge for SeniorCare Transitions by AllinaHealth.
